Joining Garden Path Elder Living
Follow the three steps below to begin your admission to Garden Path.
Step One
The first step is to fill out an inquiry form. This can be filled out by the resident, or by a family member, and gives us enough information to begin a conversation about options here at Garden Path. Once we receive it, we will be in touch to schedule a phone conversation and a tour of Bradely House.
Step Two
When you are ready to move ahead towards admission, download and review our Application, which includes detailed financial and health information. This form should be printed, filled out by both the applicant and their physician, then faxed, mailed, or hand-delivered.
Step Three
Once the application and all forms are received, we’ll schedule an appointment for an assessment with one of our nurses. Finally, we’ll review payment options with you, view available rooms, and make a plan for moving day.
